TARGET AUDIENCE:

The target audience includes primary care, internal medicine, family medicine, geriatricians, nurses, physical/occupational therapists, social workers, pharmacists, dietitians and other health professionals interested in issues related to innovative models of care for older adults with an emphasis of non-institutional care.

COURSE OBJECTIVES:

The goal of this program is to update practitioners with timely information on innovative models of care for older adults with an emphasis on non-institutional care that helps people age in place. At the completion of this program, the attendees should be able to:

  1. identify at least 3 innovative programs that contribute to keeping older adults at home and managing their chronic disease;

  2. describe programs that allow older adults to avoid institutional care while supporting a good quality of life;
  3. explain new approaches to health care delivery for older patients; and
  4. describe available options of care for their patients or clients.

COURSE DESCRIPTION:

This is a one day conference that will address timely issues and advances in the management of chronic disease and non-institutional care models that have demonstrated success.  As the population ages, providing care and services to elders living at home becomes an economic as well as medical imperative. The topics are timely and the speakers are expected to address the most up to date information that will be useful for all clinicians who care for older adults.

ACCREDITATION:

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the University of Arkansas for Medical Sciences College of Medicine and Geriatric Research Education and Clinical Center (GRECC) at the Central Arkansas Veterans Healthcare System. The UAMS College of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The UAMS College of Medicine Office of Continuing Medical Education designates this educational activity for a maximum of 5.25 AMA PRA Category 1 Credits™.  Each physician should only claim credit commensurate with the extent of their participation in the activity.

Central Arkansas Veterans Healthcare System is an approved provider of continuing nursing education by Arkansas Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

Participation in this activity provides 5.25 contact hours of continuing nursing education credit.

Application for CME credit has been filed with the American Academy of Family Physicians.  Determination of credit is pending. Application for CEU’s has also been made for dietitians, pharmacists, nursing home administrators and other disciplines.
