Purpose or Objectives: At the conclusion of this activity, the participant should be able to:
- Discuss the effects of advancing age on hospitalization and readmission for CHF.
- Describe the risk factors of acute hospital readmission in the elderly with CHF.
- Review the latest drug and non-drug therapies for patients suffering from CHF in LTC facilities.
- Understand and appreciate the nuances associated with treatment of CHF and associated conditions in elderly patients.
