Purpose or Objectives:  At the conclusion of this activity, the participant should be able to:

  1. Discuss the effects of advancing age on hospitalization and readmission for CHF.
  2. Describe the risk factors of acute hospital readmission in the elderly with CHF.
  3. Review the latest drug and non-drug therapies for patients suffering from CHF in LTC facilities.
  4. Understand and appreciate the nuances associated with treatment of CHF and associated conditions in elderly patients.
